Our three Matthew Walker Comprehensive Health Center locations serve patients in Nashville, Clarksville, Smyrna and surrounding areas. All of our sites offer primary care services. Dental services are available in our Nashville and Clarksville locations, and our Pharmacy is located at our Nashville location.

Some other things to remember:
- Bring a list of the medications and supplements such as vitamins you take.
- Note your medical history. For example, any surgeries or serious illnesses you have had, your family health history, immunization records, etc.
- If you aren’t insured and want to apply for discounted fees, bring proof of your income. This can be your W2, most recent pay stub, most recent tax return, proof of Social Security income, a notarized letter from your employer or from an assisting agency who knows your financial status or a Social Security letter.
Fee Collection
Matthew Walker Comprehensive Health Center, Inc.’s (“MWCHC”) aims to help you, our valued patient, prepare for upcoming changes to MWCHC’s fee collection practices.
MWCHC expects the changes to be fully implemented in early 2025. The goal of this update is to help you plan for the changes so that this transition goes smoothly for you.
Your Discounted Fees Will Be Due at the Time of Service. If you apply for a discount under MWCHC’s Sliding Fee Discount Program, MWCHC determines your ability to pay based on your income, household size, and the applicable Federal Poverty Level.
The amount you are required to pay is a discounted fee based on our regular charges for services (the “Discounted Fee”). Effective December 1, 2024, MWCHC requires you to pay that Discounted Fee before or at check-in for your appointment.
Payments that are Patient’s Responsibility Under Your Insurance. For any payments that are your responsibility based on your health insurance, effective December 1, 2024, MWCHC will collect the co-payment, co-insurance, and any other payments due under your policy before or at check-in for your appointment. Please also note that, if your health plan is a high-deductible plan and your deductible has not been met, MWCHC will collect the full amount due for your visit. If the full amount due for your visit cannot be fully determined at the time of check-in, MWCHC will collect an upfront payment in the amount of $50.00 (the “HD Upfront Payment”) towards any amount that is your responsibility for purposes of meeting your deductible.
Payment Plan Assistance. Please note that MWCHC understands that any patient can encounter financial hardship that creates an inability to pay your Discounted Fee, HD Upfront Payment, and potentially other fees. MWCHC is committed to ensuring patients continue to have access to care with MWCHC during a period of financial hardship. Payment plan assistance is available for those who demonstrate that they have an inability to pay as long as patients meet the requirements under MWCHC’s policy. MWCHC ensures that services are provided to all patients without regard to their ability to pay.
Refusal to Pay. To the extent allowed by law and MWCHC policy, MWCHC will require any patient to reschedule upcoming appointments if the patient has refused to pay the Discounted Fee, HD Upfront Fee, other fees, or amounts due under a MWCHC-approved payment plan. These changes that require upcoming appointments to be rescheduled due to refusal to pay are expected to be fully implemented by early 2025.

For those without insurance:
- You may qualify for discounted service and treatment fees based on your income and family size. We can help you figure this out by bringing proof of your income to your visit.
