MWCHC’s Dial Down Diabetes (DDD) Lifestyle Management Program (Nashville location) is a community-based program offered to adults that have been diagnosed with diabetes, pre-diabetes or is a caregiver to a person with diabetes. It is designed to give members the tools to “dial down” their HbAIC levels by:
• Providing diabetes education, nutrition education, and exercise opportunities to adults who are diabetic, pre-diabetic, at risk of becoming diabetic or caregivers of a person with diabetes.
• Having access to diabetic resources and health professionals so program participants that may otherwise not be obtained such as health recommendations, fresh fruits, vegetables, healthy food items from the MWCHC food bank, cookbooks for diabetics and/or diabetes magazine subscriptions.
• Increasing physical activities and setting other self-care goals.
