SUMMARY:Dial Down Diabetes - Lifestyle Management Program
DESCRIPTION:MWCHC’s Dial Down Diabetes (DDD) Lifestyle Management Program (Nashville location) is a community-based program offered to adults that have been diagnosed with diabetes\, pre-diabetes or is a caregiver to a person with diabetes. It is designed to give members the tools to “dial down” their HbAIC levels by: \n• Providing diabetes education\, nutrition education\, and exercise opportunities to adults who are diabetic\, pre-diabetic\, at risk of becoming diabetic or caregivers of a person with diabetes. \n• Having access to diabetic resources and health professionals so program participants that may otherwise not be obtained such as health recommendations\, fresh fruits\, vegetables\, healthy food items from the MWCHC food bank\, cookbooks for diabetics and/or diabetes magazine subscriptions. \n• Increasing physical activities and setting other self-care goals. \nIf you are already a participant in the DDD Program\, please RSVP at least two days prior to the meeting date.\nIf you have NOT signed up\, please call or email so you can join us! \nTracye Henderson\n615-324-9668\ntracye.henderson@mwchc.org \nEmily Germer\n615-327-9400\nemily.germer@mwchc.org

DESCRIPTION:Matthew Walker Comprehensive Health Center\, Inc. (MWCHC) is very excited to be celebrating 55 years of service! Our legacy and commitment to provide health care has proven to be unshakable. We were born out of the civil rights era\, grown through years of cutbacks\, and we were leaders through the pandemic. \nOur signature event EPIC – Entertainment\, Philanthropy and Involvement in the Community will be held on Friday\, October 20\, 2023\, at GEODIS Park. We will have a night full of fun\, while we raise funds to further our mission to provide quality health care services\, health education and to promote wellness. Our fundraising goal for EPIC is $250\,000. Funds raised through EPIC will help us continue to answer the call to provide health care services and programs regardless of a person’s insurance or income status.
