If you’re uninsured, you may qualify to pay a reduced price for some services and treatments at our health centers. The exact amount you pay depends on your income and family size. This is the “Sliding Scale”. To qualify, you must bring in proof of your family’s income.
Appt Request

We encourage patients, new and established, to make appointments at any of our three locations. As

a courtesy, you can now request appointments via our website! Please fill out the form below in its entirety. Our Appointment Desk will contact you back within 48 hours to confirm your requested appointment or to help find an alternate appointment time that is convenient for you.

Established patients are welcome to specify a preferred provider (although it is not required) by entering his/her name in the section entitled Physician Name (Optional) below. All new patients will be assigned to the physician that is scheduled to be on duty during the requested appointment time (unless a provider is specified).
